Later, Sara receives a letter Michael wrote before the break: “If you’re reading this, I’ve done what I set out to do. You’re safe. That’s all I ever wanted. Tell our child about me — not the escapes, not the prisons. Just that I loved you both more than I ever loved freedom.” Sara is crying, holding a sonogram. She’s pregnant. Four years later, Sara and Lincoln live in Panama. Little Michael (named after his father) plays on a beach. Lincoln runs a small boat repair shop. Sucre visits often. Mahone has disappeared into witness protection — alive but haunted.

As they flee, Michael starts bleeding from the nose — a sign of the brain tumor that was supposedly removed earlier. He hides it from Sara. The group reaches a dock where a boat waits to take them to Mexico. But prison helicopters are minutes away. Michael realizes the boat is too slow. Sara hears this and breaks down. Michael kisses her one last time, whispers “Just live,” and swims toward the power junction. The prison boats approach. Michael connects the cables. The night before Sara’s execution, Michael puts the plan in motion. He short-circuits a generator, causing a blackout. Guards scramble. He crawls through a series of steam tunnels, avoiding patrols and razor wire. Meanwhile, Lincoln creates a diversion outside: a fake truck crash near the prison wall. Sara, in her cell, hears three knocks on the floor — Michael’s signal. She moves her bunk and finds a loose grate. Michael reaches up from below. Their hands meet.
