Jamie follows the ribbon clue to a tavern called The Whip & Hind. He strong-arms a sailor into revealing Leonard’s hideout: a cave on the Isle of May. Jamie hires a boat and sets off alone, armed only with a dirk and a lantern.

On the island, Jamie finds Young Ian in a holding cell. Leonard appears—a sadistic English deserter. A brutal knife fight ensues. Jamie is wounded in the side but kills Leonard. In Boston (1968), Claire (Caitríona Balfe) is now a top cardiothoracic surgeon. She’s dating a fellow doctor but remains emotionally distant. At a medical conference, she sees a vintage print from Edinburgh’s Carfax Close—and recognizes the name “A. Malcolm” as Jamie’s alias. Meanwhile, in the 20th century, Claire continues her surgical career and uncovers a clue that could change everything. Full Scene-by-Scene Breakdown 1. Cold Open – The Cave Flashback to 1746, post-Culloden. Jamie hides in a cave, alone and haunted by the ghosts of fallen Highlanders. He writes letters to Claire he knows he’ll never send. A rat scurries—he kills and eats it. This sets his survivalist, fractured mental state. On the island, Jamie finds Young Ian in a holding cell

Jamie (Sam Heughan) is now a partner in a small print shop. He lives with his dour nephew, Young Ian (John Bell), and their loyal dog, Gideon. At night, Jamie smuggles brandy. By day, he prints seditious pamphlets. His identity as “Alexander Malcolm” is wearing thin.

Young Ian is snatched from the docks by armed men working for a pirate named Thomas “The Rake” Leonard. Jamie is knocked unconscious. He wakes to find a single clue: a piece of green ribbon from Ian’s shirt.
